ANY IDENTIFYING INFORMATION OF THE PATIENT, SUCH AS NAME OR MEDICAL RECORD
NUMBER.
Submission Instructions:
1. First, give a copy of all documents above to your preceptor during your
meeting. Some preceptors may also like to receive a copy of these prior to the
meeting so they may review ahead of time and be more efficient at the time of
the meeting.
2. Meet with your preceptor to discuss your write-up.
You may wish to bring a blank copy of the preceptor evaluation form with
you for guidance on the discussion.
Your preceptor should complete the evaluation on line while you are together and
send you a digital copy. (The preceptor will have access to a digital form.
Digital is much preferred,
however, handwritten evaluations are also accepted (i.e.
scanned and uploaded as an attachment into Sakai). Your preceptor
should keep a copy for their records.)
3. You do not need to re-write the H&P or other forms after the review. Comments and corrections provided by your preceptor are for your skill development. Use these comments to improve upon the next write-up.
4. Submit the H&P, all additional requirements listed and your preceptor’s evaluation form into Sakai by the due date. The H&P write-up and evaluation are two separate assignments, listed in Sakai.
You are responsible
for submitting all these items on time, including the preceptor evaluation,
therefore, arrange meetings well ahead of time and ask the preceptor to complete
it either while you are together or as soon as possible.
