Log cards
After your orientation you picked up yellow log card. Please transfer your data from the card to the computer often. Please make sure you log all your patients/procedures online. If you were not able to see patients with specific required conditions, please complete alternative cases listed in below table.

Modus operandi
Enter data on-line/myLumen. It is anticipated that you will use the card to collect data and enter the final total numbers in the computer application. You will have access to the "student log" application from myLumen.

Competency
Failure to complete the card in the computer application on time will be flagged as "concern" for Professionalism competency or points deduction from your final grade.
