Registration Activities
During Monday and Tuesday afternoon of Orientation, you will be
expected to find some of the campus offices on your own and complete
your registration activities. They include:
First Year Registration (during Monday morning’s
presentation)
In the event that it is not possible for you to register on Monday,
July 28, you must contact Dean Jones in the Admissions Office
before June. Please understand if you do not appear in person for
registration, or make satisfactory arrangements in advance, your
place in the class may be given to a person on the Alternate List
at noon on Monday, July 28, 2008.
Parking and Student I.D. Office (Mulcahy Center, Room
1606)
The Parking and Student ID Office is located in the Mulcahy Center,
Room 1606. You will receive a Parking Registration Form during Orientation.
Please be sure to have your automobile make, model, year and license
plate number available in order to complete the form. You will be
required to bring the completed form to this office during the second
day of Orientation in order to obtain a parking key card and hang
tag for the student parking deck. Payment of $253 will be required
at that time. You will also be required to sit for a photograph
to be used on your Student I.D.
Student (and Occupational) Health Service (1211 W. Roosevelt
Road)
You must update and verify all immunizations and your health history.
Any immunizations that are missing must be administered before you
can begin class. These immunizations (or titers) can be administered
in the Student Health Service. If you are missing any required immunizations,
you will be required to receive and PAY for them at the time of
service, and before the beginning of class. The immunizations can
cost anywhere from $15 to $60 per shot, with titers running $18
to $103.
Health And Hospitalization Insurance
Loyola University Chicago (including the Stritch School of Medicine)
requires each of its full-time students to be covered by an adequate
health insurance plan. The student may select any plan such as one
provided by a spouse or parent. If the student is not covered under
another plan, or fails to provide timely proof of same, then the
University sponsored plan offered through Unicare/HTH must be purchased.
To verify the insurance status of all students, the University has
a "hard waiver" insurance plan. The hard waiver policy
mandates that every full-time student either provide their current
insurance information (and attest that their insurance will be maintained
through the entire academic year) or automatically become enrolled
in the University plan.
The premium for the plan will be added to all student tuition bills.
If you wish to waive out, you must complete the on-line waiver form
no later than Friday, August 1st. You will be asked pertinent information
regarding a current carrier (such as name of company, policy number,
group number, membership number), and you will be asked to electronically
sign the waiver indicating this other insurance will be in place
the entire academic year. As we receive confirmation of a waiver
from the insurance company, we will credit your account. If you
desire to be enrolled in the plan, all you must do is submit payment
to the Bursar at the same time your tuition and fees are due. Any
student who fails to meet the deadline will automatically be enrolled
in the plan and MUST pay for it regardless of whether or not they
have other insurance. This deadline is firm; there will be no exceptions
or extensions.

Proof of Citizenship
Registration and Records (SSOM, Room 220)
In order to complete final registration, all entering students,
whether U. S. citizens, non-citizens, U. S. citizens born on foreign
soil or naturalized citizens, must provide original citizenship
verification documents when they register during Orientation
in late July.
U. S. Citizens: Provide your original Social Security
Card or valid U.S. Passport
Naturalized Citizens: Provide your original naturalization
certificate.
Non-Citizens: Present your original Permanent
Resident Visa Card or necessary documentation from the United States
Immigration and Naturalization Service (visa or passport).
All students in the Entering Class who have a Permanent Resident
Visa must submit a photocopy of the valid Permanent Resident Visa
Card to the Admissions Office no later than Monday, July 14, 2008.
The photocopy must include both the front and back of the card.
Please contact the Admissions Office if you have any questions concerning
this matter. In addition, original proof of citizenship
will be required at the time of registration.
Please review these directions carefully and be sure to bring
the necessary original documents with you during
orientation. We will be unable to complete your Stritch School of
Medicine registration without these documents.
Medical Photography (SSOM, Room 340)
You must sit for a photograph that will be used for the class composite
for faculty and staff use.
Long Term Disability Income Insurance
We are pleased to inform you that as a medical student at Stritch
School of Medicine Loyola University Chicago you will be covered
by a personal disability insurance policy. This coverage
is mandatory for all medical students. (You may complete
the form by using the link below. We will also provide copies during
Orientation.)

Financial Aid and Bursar’s Offices (SSOM, Room 210)
During Orientation you will be required to meet with the Bursar
and Financial Aid staff to go over any outstanding loan package
issues.
Criminal Background Clearance (SSOM, Room TBA)
The State of Illinois has required all Illinois medical schools
to perform both a fingerprint-based criminal history records check
for violent felony convictions and an inquiry of the Department
of State Police Sex Offender Registration Database on all matriculants.
The background check is done to comply with state law, with the
intent to ensure that students with patient contact will be providing
care with the highest level of safety and integrity; to comply with
applicable healthcare regulations and requirements of other healthcare
institutions; and to meet emerging accreditation standards. The
screening will determine whether there are incidents in an applicant’s
history, such as violent criminal behavior, that would pose a risk
to patients or others, or would prohibit the individual from being
licensed in the future. During Orientation we will have representatives
from the company we have contracted to process the fingerprint collection
and submission.
