Loyola Center for Health & Fitness

Every medical student receives a membership to the Loyola Center for Health and Fitness, which you probably visited during your interview and tour of the campus. During Orientation you will be required to fill out and return the attached Health History Questionnaire (link below).

You will also be required to complete a Membership Agreement Form, which will be provided during Orientation.
