Fellowships CA4 Year

Opportunity exists for a limited number of residents to pursue a fellowship year after three years of clinical anesthesia. Fellows may elect to spend their time either in basic research or in one clinical area to master the clinical subspecialty. The clinical fellowships offered include pain management, and cardiovascular and thoracic anesthesia. The fellows are expected to participate or initiate clinical research during this year which should result in more than one publication.

 

Pain Management Fellowship Program

The Department of Anesthesiology offers a twelve month fellowship for CA-4 (PGY-5) trainees. This program is accredited by ACGME. Successful completion of this fellowship,along with ABA certification and current state licensure will make the candidate eligible to take the ABA Pain Management examination.

The fellowship aims to train an anesthesiologist in the art and science of pain management. Included in the definition of pain syndromes are acute (post-operative), chronic, and oncologic pain. Techniques which the trainees will become familiar with include: nerve blocks, fluoroscopic-assisted nerve blocks, epidural steroid injections, spinal cord stimulators and implantable pumps, peri-spinal opioid therapy, use of various medications such as opioids, anti-depressants, anti-convulsants, and non-steroidal anti-inflammatory agents. Other modalities, with which the fellow will become familiar, are bio-feedback, physical and occupational therapy, exercise therapy, psychotherapy and complementary medicine.

During the training, diagnosis and management of pain syndromes are stressed. As the trainees will quickly realize, nerve injections rarely, if ever, cure chronic pain. "Curing" chronic pain is not always possible, rather, helping the patient control the pain to allow increase function by utilizing a multidisciplinary approach is our common goal. The various modalities listed above allow us to help manage chronic pain. Allied disciplines to which the trainee is exposed include: psychology, radiation oncology, medical oncology, radiology, physical and rehabilitation medicine, physical therapy, and occupational therapy.

During the year the fellow will spend most of his time in the Pain Management Center, located in the Loyola Outpatient Center. The fellow also has the opportunity to see patients at the Loyola Cancer Center, and at the Oakbrook Terrace Outpatient Clinic.

He/she will be expected to cover the Anesthesia Pain Service one to two weeks per month, including two weekends per month on beeper call. Attendance at one or two pain management/regional anesthesia review courses is strongly encouraged during the year. Examples are the American Society of Regional Anesthesia (ASRA) Annual Meeting and Review Course (usually held in March), the ASRA Pain Management Review course,The American Pain Society Annual Meeting (usually held in November), and the International Society for the Study of Pain (held bi-annually).

Participation in on-going clinical or laboratory research in an area related to pain management or regional anesthesia is a required part of the fellowship. The fellow will be expected to submit at least one scientific abstract for publication during the fellowship.

Cardiovascular and Thoracic Anesthesia Fellowship Program

Loyola University offers the cardiothoracic anesthesia fellow a high-quality clinical and didactic experience which provides excellent preparation for future practice, whether in academics or in the private sector. Our cardiothoracic anesthesia fellows are trained by a dedicated staff of cardiothoracic anesthesiologists, and also receive the benefit of the vast and varied experience of our cardiothoracic surgeons, cardiologists and pulmonologists.

Our clinical caseload is outstanding in diversity, number and acuity of care. Loyola University is the most active heart and lung transplant center in Illinois, and the fellow will gain expertise in the management of patients with end-stage heart failure and lung disease, performing anesthesia for heart transplants, lung transplants and implantation of ventricular assist devices. The fellow will obtain extensive experience in the management of a variety of complex cardiac and thoracic surgical cases, including reoperations, multiple valve operations, thoracic aortic surgery, correction of congenital anomalies, as well as newer Robotic mitral valve repair and CABG and thoracoscopic surgery. Our department provides anesthesia for well over 1000 cardiac and thoracic cases per year.

Fellows obtain extensive training in transesophageal echocardiography. While this training occurs primarily in the operating room, the fellow also spends time working directly with our cardiologists in a one to two month rotation outside the operating room, improving diagnostic and interpretive skills.

Experience in the postoperative critical care of cardiac patients is obtained during rotation in the post-cardiac surgical intensive care unit with supervision and teaching by our own anesthesia faculty. This allows the individual to gain a greater appreciation of the entire perioperative course as an extension of our anesthesia care. Close contacts with consultants from other specialties strengthen the educational benefit of this rotation.

In addition to intraoperative teaching, we have a didactic program consisting of journal clubs and weekly lectures on advanced cardiac and thoracic topics. The fellow will also have access to a wide range of multimedia educational materials in transesophageal echocardiography. Past fellows have elected to be involved in clinical only or combined clinical/research programs.

Our facility is state-of-the-art. We have three dedicated cardiothoracic operating rooms, each equipped with transesophageal echocardiography machines and omniplane probes. Continuous cardiac output monitoring, mixed venous oximetry, ECG ST segment analysis, electroencephalography, nitric oxide and pressure control ventilation are readily available.
